Cutting No-Shows: Getting Scheduled Patients Through the Door

A screening no-show wastes a staff block and a candidate. Most no-shows are preventable with reminder design and friction removal.

Every scheduled screening visit that ends in a no-show burns a room, a coordinator hour, and often the candidate — many no-shows are too embarrassed to rebook. Sites tolerate no-show rates of 20–30% as if they were weather. They're not.

Why people no-show

Surveys and call-backs point to a consistent list: they forgot; something came up and rescheduling felt hard; they got cold feet about the study; or logistics (transport, childcare, work) collapsed. Each cause has a different fix, which is why "send more reminders" alone plateaus.

The playbook

Confirm at booking. Send a calendar invite plus a text confirmation immediately. People who add the visit to their own calendar show up more.

Remind on a 3-1 rhythm. A reminder three days out (with a one-tap reschedule link) and one the day before. The reschedule link is the crucial part: it converts silent no-shows into rebookings by making the honest option easy.

Reduce the unknowns. Fear of the unfamiliar drives cold feet. A short "what to expect" message — where to park, how long it takes, what happens at the visit, that they can ask questions before anything is done — measurably improves show rates, especially for research-naive candidates.

Solve logistics proactively. Ask at scheduling: "Anything that might make getting here difficult?" Ride services and flexible slots cost far less than empty ones.

Call the high-risk bookings. Leads who were hard to reach initially, or who booked more than two weeks out, deserve a personal call the day before. Long lead times are no-show incubators; keep the gap between "yes" and the visit as short as the calendar allows.

Sites that run this full sequence routinely operate below 10% no-show. The difference between 25% and 10% is, for most sites, several enrollments a month — from candidates they had already won.
