Why 80% of Clinical Trials Miss Their Enrollment Deadlines

Enrollment delays are the single biggest driver of trial timelines slipping. The causes are predictable — and mostly fixable at the site level.

Roughly four out of five clinical trials fail to meet their original enrollment timeline, and about a third of trial delays are attributed directly to recruitment. For research sites, that statistic isn't abstract: it shows up as extended timelines, frustrated sponsors, and studies that quietly lose funding.

The usual suspects

The reasons trials under-enroll are remarkably consistent across therapeutic areas:

  • Overly optimistic feasibility estimates. Sites project enrollment from their total patient database, not the fraction who actually meet inclusion criteria and will say yes.
  • Slow first contact. A lead who fills out a web form and hears nothing for five days has usually moved on. Speed-to-contact is the single strongest predictor of conversion in most site datasets.
  • Protocol complexity. Every additional inclusion criterion shrinks the eligible pool, often more than sponsors anticipate.
  • No systematic follow-up. Most sites contact a lead once or twice. Interested-but-busy patients fall through the cracks without structured cadences.

What high-enrolling sites do differently

The sites that consistently hit their numbers treat recruitment as a pipeline, not a task. Every referral enters a tracked funnel with a defined status. Someone owns each stage. Response-time targets are measured in hours, not days.

They also pre-screen against their existing database before spending on outreach. A registry of past participants and previous screen-failures — with consent to re-contact — is usually the cheapest source of enrollment a site has, and the most neglected.

The takeaway

Enrollment problems are rarely solved by spending more on ads. They're solved by converting more of the interest a site already generates: faster contact, structured follow-up, and honest feasibility math before the study starts. Fix the funnel first; scale the spend second.
