Working With CROs: A Site’s Guide to a Smoother Relationship

CRO-site friction is legendary and mostly avoidable. A few habits at study start prevent the majority of mid-study pain.

Sites rarely choose the CRO — but they live with the relationship for the length of the study. The sites that manage it deliberately spend less time fighting queries and chasing payments, and more time enrolling.

Set the terms at start-up

Negotiate the recruitment plan like it's binding — because it is. Enrollment targets, screen-fail expectations, and advertising budgets agreed at start-up become the yardstick you're measured by. Push back on feasibility fantasies before signing; "we'll figure it out" becomes "the site is behind" within two quarters.

Get payment terms specific. Per-completed-visit payment schedules, screen-fail reimbursement caps, and invoice timelines deserve as much scrutiny as the budget total. Cash-flow surprises, not budget totals, are what actually hurt sites.

Clarify communication channels. One CRA email thread, a portal, and a weekly call is workable. Four portals and ad-hoc emails from rotating contacts is not. Ask who your stable point of contact is and what happens when CRAs turn over — because they will.

Manage the middle

Report enrollment reality early. CROs escalate surprises, not problems. "We're two behind plan because screen-fails are running 45% on the lab criterion — here's our mitigation" keeps the site in the driver's seat. Silence followed by a missed milestone invites micromanagement.

Document query patterns. If monitoring visits repeatedly flag the same data point, fix the workflow, note the fix, and say so. CRAs write reports; give them good material.

Track your own performance metrics. Sites that arrive at close-out with their own numbers — enrollment vs. plan, query rates, data-entry timelines — negotiate the next study from strength.

The long view

CRO project managers move between companies and remember sites. Being the site that communicates early, documents well, and hits realistic commitments is business development you don't have to pay for.
