Negotiating Recruitment Budgets That Reflect Reality

Sites routinely accept recruitment budgets built on optimistic assumptions, then eat the difference. Better numbers at negotiation prevent it.

Recruitment line items get less negotiation attention than procedure costs — and cause more grief. The typical failure: a budget assumes participants will materialize from the site's database, the database yields half of them, and the site funds the rescue advertising itself.

Know your real acquisition costs

Before any negotiation, a site should know from its own attribution data:

  • Cost per enrollment by channel, from prior comparable studies
  • Realistic database yield for this protocol (run the actual criteria — see feasibility)
  • Coordinator hours per enrollment: calls, screens, no-shows, and re-screens included
  • Historical screen-fail rate for similar populations, and what a failed screen costs

Sites without these numbers negotiate against sponsors who have industry benchmarks. That asymmetry is expensive.

Structure the budget defensively

Separate the recruitment fee from advertising pass-throughs. Coordinator effort is a fee; media spend is a reimbursable with receipts. Blending them means media overruns eat your labor margin.

Tie advertising triggers to the funnel. "If database and referral channels yield fewer than X screens by week 8, sponsor-funded advertising activates at $Y" — pre-agreed, so the mid-study rescue doesn't require a renegotiation from weakness.

Price screen failures honestly. A capped screen-fail reimbursement below your real cost, multiplied by a 35% fail rate, is a hidden discount you're granting. Do the multiplication in the negotiation, out loud.

Include re-consent and rescreening events. Amendments happen; the labor they trigger shouldn't be free.

The posture

None of this is adversarial — it's the same funnel math sponsors respect in feasibility, applied to money. The site that says "here's our cost model, line by line" is easier to fund than the one that says "that seems low." Precision reads as competence, and competence gets budgets approved.
