Recruitment Seasonality: Planning Around the Calendar

Enrollment doesn’t flow evenly across the year. Sites that plan for seasonal patterns keep their funnels full through the troughs.

Every experienced coordinator knows the rhythm: the December cliff, the January surge, the summer sag. Yet most recruitment plans assume a flat calendar and then act surprised twice a year.

The predictable patterns

Holiday season (mid-November through New Year): response rates fall, scheduling stalls, and no-shows climb as routines dissolve. Enrollment pushed into this window costs more per participant than any other time of year.

January through March: health intentions peak. New-year resolve, deductibles resetting, and post-holiday routine produce the year's most responsive audience for many indications.

Summer: family schedules and travel produce a softer, choppier version of the holiday dip — worse for pediatric studies, milder for retiree-heavy populations.

Condition-specific seasons: allergy studies live and die by pollen calendars; SAD and flu studies have obvious windows; even chronic-condition studies see symptom-driven inquiry spikes.

Planning around it

Front-load before troughs. If a study activates in October, push hard immediately — the leads you bank before Thanksgiving carry you through the dead weeks.

Use troughs for infrastructure. December is a poor month for cold outreach and an excellent one for database hygiene, registry re-engagement drafts, IRB submissions for Q1 campaigns, and referral-network thank-yous.

Time the registry blast. A "new studies this year" message in the first week of January lands on the year's most receptive audience — the same message in late December disappears.

Budget unevenly. A flat monthly ad spend buys expensive leads in troughs and misses cheap ones in peaks. Shift spend toward high-response windows.

Seasonality is only a problem when it's a surprise. Written into the enrollment plan — with sponsor expectations set accordingly — it becomes one more variable a well-run site simply manages.
